We are pleased to offer you a convienient, online method to apply for financial aid through FACTS for Faith Lutheran Middle School & High School.
Simple Steps to Apply for Financial Aid Online:
1. Choose "Create a username & password". If you have an existing FACTS account and have already created a user account, please select "Sign In" and use your existing username and password.
2. Click on the "Start Application" link.
3. Select the appropriate school year.
4. Complete the steps as prompted.
A Message from your School: Applications for tuition assistance must be submitted
annually online after January 1st for the following school year.
Families will be notified via email of assistance award decisions once the
application review has been finalized on completed applications. All
tuition assistance awards are subject to funds available.
PLEASE NOTE: Make sure to upload or
fax all required tax documents for the applicant & co-applicant including
W-2’s, Form 1040 Tax Return (2 pages) and required Schedules (C, E, F), Form
4562, along with all Business Returns that are listed on your Schedule E (Form
1065 – 5 pages, Form 1120S – 5 pages, Schedule K-1 and Form 8825), and most current copy of checking and savings account statements. If you
do not file a tax return you must provide proof of all household
Non-Taxable Income received such as Social Security, Food Stamps,
Welfare, Child Support, TANF and Workers’ Compensation.
